| GATEWAYS COMMUNITY
LIVING PROGRAM
Catholic Charities of Chemung County’s
Gateways Community Living Program is a unique delivery
of supportive housing assistance. The only program of
its kind in our area, Gateways works to ensure the housing
needs of the mentally ill; HIV or chemically dependent
individuals are achieved. This program delivers varying
degrees of supervision and education. At Catholic Charities
Gateways Community Living we protect the dignity and
well being of everyone we serve…and we serve all
faiths!
Gateways Community Living Program
is not a mental health program and does not provide
clinical services. We do provide quality case management
and residential support services. The case management
process begins by identifying strengths, weaknesses
and needs of each person. Then personal goals are set…ranging
from basic living skills such as cooking, cleaning,
independent travel and shopping. More advanced goals
may include managing budgets, taking vocational classes,
and seeking and maintaining employment.
The ultimate goal of the Catholic Charities
Gateways Community Living Program is to advance all
clients toward self-sufficient living in a safe and
secure environment.
Supervised Community Residence
- Our supervised residence is home to individuals with
serious and persistent mental illness. These individuals
require 24 hour supervision, medication management and
intensive case management. We teach basic fundamental
living skills and involve our residence in cooking,
cleaning, shopping, and the everyday activities of running
a home.
Sunshine Apartments
- Catholic Charities Sunshine Apartment program offers
shared apartment living for individuals who are seriously
and persistently mentally ill yet possess more advanced
living skills than those in our Supervised Community
Residence. People at this level of care have sufficient
skills to manage on their own with intensive case management
and support services
Supported Housing
- Catholic Charities Supported Housing program is a
subsidized housing option offering financial assistance
to mentally ill individuals who can live independently
in the community. Along with financial assistance we
offer needed support, encouragement, and guidance. Case
management is provided depending on individual needs.
Shelter Care Plus
- This program is similar to our Supported Housing program
in client skill level, case management, and support
services offered. Shelter Care Plus provides subsidized
housing to individuals who are homeless and have a diagnosis
of mental illness, HIV, or chemical dependency.
